A Clinical Observation of the Benign Surgical Diseases of the Biliary System


Abstract
This study is a review and analysis of 60 cases of benign surgical diseases of the biliary system treated at Department of Surgery, Chungnam National University Hospital from July 1972 to June 1975.
The results were as follows:
1) Among 60 cases, the causative diseases were cholecystitis with stone 34 cases (56.7%), cholecystitis without stone: 7 cases (11.7%), empyema of gallbladder: 10 cases (16.7%), perforation of gallbladder: 5 cases (8.3%), acute cholangitis: 2 cases (3.3%), congenital anomaly: 1 case (1.7%), trauma: l case(1. 7%).
2) The incidence was high in 3150 years and average age was 48. Male was more prevalent than female.
3) The occurrence was more in winter than in other season.
4) The duration from the first attack to admission was within 1 year in 23 cases (38.3%), 1-5 years in 15 cases (25%) and over 5 years in 21 cases (35%).
5) The prominent symptoms and signs were pain at R.U.Q. of abdomen; tenderness, jaundice, fever and chillness, indigestion, nausea and vomiting, hepatomegaly etc. in order of frequency.
6) The rate of positive finding, in T-tube cholangiogram was 32.5% (13 in 45 cases)
7) Operation procedures were as follows: cholecystectomy and choledochostomy .33 cases (55.0%), cholecystectomy: 10 ccases.(16.7%), cholecystostomy: 8 cases (13.3%.) and others 9 cases.
8) Thirty four cases had stones in C.B.D. and 16 cases in intrahepatic duct (72.4% and 34%). 9) Postoperative complications were wound infection in 5 cases (8:3%), Residual stones in C.B.D. in 4 cases, bile leakage in 2 cases etc.
10) The operative mortality was 8.3% (5 cases) and septicemia was the cause of death in most of them.
